Employer Forms
Employer/Student/Faculty Agreement Form is to be completed together by:
1). The Employer Internship Supervisor & Student Intern
2). The Faculty Advisor & Student Intern
Please follow the instructions below:
- STEP 1: The student must individually complete all yellow shaded areas.
- STEP 2:The student and Employer Internship Supervisor together must discuss goals & objectives and complete the blue shaded areas. Goals and objectives must clearly state what the student will be learning as a result of their internship job responsibilities.
- STEP 3: Student must meet with their Faculty Advisor to complete the remaining gray shaded areas. Please be sure you are aware of your academic department's guidelines/requirements for internships.
- STEP 4: The signature of each party is required. Employer Internship Supervisor & student electronic signatures will be accepted. A Faculty Advisor signature must be handwritten last to verify agreement of goals /objectives.
- STEP 5: Students must return this form to the Career Center along with the Add/Drop Form.
- STEP 6: The Career Center will stamp agreement form and Add/Drop form for credit registration.
*Please note, a faculty member will be contacting you via phone to discuss the student’s internship progress.
Internship Program Employer-Student Evaluation Form must be completed by:
1). The Employer Internship Supervisor
2). The Student Intern
Please follow the instructions below:
- STEP 1: Please complete the form approximately 2/3 of the way through the internship experience.
- STEP 2: Signatures of both the Student Intern and the Employer Internship Supervisor is required.
- STEP 3: Return the form to the Career Center.
NOTE: Faculty, Student & Employer will be responsible for printing a copy of all forms for their files. The Career Center will continue to send Faculty the Employer Evaluation Form. Faculty, Student & Employer will receive notification from the Career Center once all paperwork has been approved and delivered to the Registrar.
