Faculty/Student/Employer Agreement Form must be completed together by:
1). The Employer Internship Supervisor & Student Intern
2). The Faculty Advisor & Student Intern
Please follow the instructions below:
- STEP 1: The student must individually complete all yellow shaded areas.
- STEP 2: The student and Employer Internship Supervisor together must discuss goals & objectives and complete the blue shaded areas.
- STEP 3: Student must meet with their Faculty Advisor to complete the remaining gray shaded areas.
- STEP 4: The signature of each party is required. Employer Internship Supervisor & student electronic signatures will be accepted. A Faculty Advisor signature must be handwritten last to verify agreement of goals /objectives.
- STEP 5: Students must return this form to the Career Center along with the Add/Drop Form.
- STEP 6: The Career Center will stamp agreement form and Add/Drop form for credit registration.
*Please note, a faculty member will be contacting you via phone to discuss the student’s internship progress.
Add/Drop Form is to be completed together by:
1). The Faculty Advisor & Student Intern
Please follow the instructions below:
- STEP 1: The student must complete and sign the Add/Drop Form.
- STEP 2: The Faculty Advisor must sign form.
- STEP 3: The student must return form to the Career Center, along with Employer/Student/Faculty Agreement Form
- STEP 4: This form will be approved by the Career Center and delivered to the Registrar so the student will be registered for academic credit.
Student Internship Code of Conduct must be completed by:
1). The Student Intern
Please follow the instructions below:
- STEP 1: Please complete and sign.
- STEP 2: Please return to the Career Center with all other internship forms.
Internship Program Student-Employer Evaluation Form must be completed by:
1). The Student Intern
Please follow the instructions below:
- STEP 1: Please complete the form approximately 2/3 of the way through the internship experience.
- STEP 2: Return the form to the Career Center with all other internship forms.
News Release Form is to be completed by:
1). The Student Intern
Please follow the instructions below:
- STEP 1: The student must complete the form.
- STEP 2: For your press release to appear in your hometown newspaper, please return form to the Public Relations Office.
NOTE: Faculty, Student & Employer will be responsible for printing a copy of all forms for their files. The Career Center will continue to send Faculty the Employer Evaluation Form. Faculty, Student & Employer will receive notification from the Career Center once all paperwork has been approved and delivered to the Registrar.
