Blackboard Course Request Form
This form is for FACULTY to use to request their course be set up in Blackboard. Please allow at least 5 business days to complete this request.
Use this form to inform IS of any specific hardware, software, e-mail, listserv, or network technology requirements you have regarding your class/classroom. Consider things you need for your classroom as well as things your students may use in a public lab. Please allow at least 5 business days to complete this request.

Video Game System Network Configuration Form
Use this form to request your Video Game System (Xbox, Play Station, etc.) be registered for use on the network. Please allow at least 3 business days for your request to be honored.
Wireless Router Registration Form
BEFORE you connect your router to the SVC network it MUST be configured by the IS department. Print this PDF form, complete all pertinent information and bring it, along with your router (including power cords, software, etc.), to the Help Desk located on the ground floor of Alfred Hall. Please allow at least 4 business days for your router to be configured.
